Provider service area gaps

If a CMHW provider is interested in adding a county to their service authorizations, please complete the following steps:

Visit the provider page on the Indiana CMHW website here > https://www.in.gov/fssa/dmha/youthservices/provider-information/
Under “Provider Approval Forms,” download and complete the application cover sheet.

Fill out the contact information at the top of the form.
Skip Section A.
In Section B, check the box for “Update Demographic Information.”
Write in “Add Counties.”
Write in the name of the employee and what county/ies you want to add. (Ex – Jane Smith, Marion County, Johnson County)

If you need additional room, the form has a link to access a provider summary. Use the provider summary to write in the counties to be added to each employee.

Service definition

All providers are expected to adhere to the DMHA Youth Services Home and Community-Based Wraparound Services Provider Agreement and the Indiana Health Coverage Programs that outlines the terms of service, including the scope of services provided, quality standards, and any other relevant details for the provider to deliver.

These agreements stipulate that the provider will comply, on a continuing basis, with all federal and state statutes and regulations pertaining to the DMHA and IHCP, as well as the standards and requirements of the 1915(i) CMHW Services Program. By signing the IHCP and DMHA provider agreements, the provider agrees to the policies and expectations provided in the IHCP provider reference modules and this module, as amended periodically, as well as all related provider bulletins and notices.

CMHW continuing education courses for Habilitation, FST, and Respite providers

The CMHW team has developed a series of continuing education courses available to HAB, FST and Respite providers to help meet professional development policy requirements. These courses are optional and NOT required. You may take each course only ONE TIME per reauthorization cycle. A few spots are still available for November and December trainings.

Supervisors: New process for adding staff virtual training

Beginning Dec. 16, 2024, Hab/FST/Respite agencies must follow a new process to add authorized providers to their agency.

The CMHW team and partners at Roeing will offer two training dates for the new process. This online training will be a live walk-through of the process with detailed instructions and opportunities to ask questions. You only need to attend one of the two training sessions. Training will also be recorded and made available, along with an instruction manual, for those who cannot attend.

Who should attend: Hiring manager(s) or the people in your agency responsible for submitting resumes and applications to DMHA.

Medicaid eligibility reminder

To ensure the participant has active Medicaid, it is best practice for providers to check Medicaid eligibility on the first and 15th of each month.

New policies
