A Healthier Hamilton County: Systems of Care provides a platform for a coordinated and collaborated effort, inclusive of monthly collaborative meetings, community education and wellness development, leadership addressing suicide in our community, and creating collaborations to meet gap needs as identified.
All in all, HHC:SOC serves Hamilton County as a whole, facilitating and participating in cross-system partnerships that provide a service outside of traditional wellness services. The programs/initiatives provided by HHC:SOC are the results of the creation of these collaborations to meet those gap needs identified.
